NorthPoint Health & Wellness accepts verified commercial insurance coverage. You may be asked to pay a co-pay at the time of service. Please present your ID card and provide information when scheduling your appointment, or at the time you register.

Managed Care Plans

NorthPoint Health & Wellness participates in many managed care plans. You will be billed for any balance identified by your health plan. Please present your plan ID card and provide information when scheduling your appointment, or at the time you register.

How Do I Apply?

The Sliding Fee fee scale application must be completed along with the required proof of income and family size. The following documents are acceptable proofs of income:

  • Most recent income tax return or W-2
  • Two most recent pay stubs
  • Most recent unemployment check
  • Proof of other household income (Social Security, pension, etc.)
  • Bank statements showing direct deposits

The documentation should be completed and submitted to one of our MNsure Navigators or Financial Case Aides. Incomplete applications will not be considered for approval.

How Much Will I Owe?

Eligibility for NorthPoint’s Sliding Fee Discount Program is based on household size and household income. Patients with a documented annual income of 200% or below the Federal Poverty Level (FPL) are eligible for a sliding fee discount on all clinic services. NorthPoint will not turn anyone away based on their inability to pay.

How Do I Qualify?

Your total household income must be 200% or less than the Federal Poverty Guidelines to be eligible for the sliding fee scale. Sliding fee eligibility is based on household size and household income.

Can I Use The Sliding Fee Scale Instead of My Insurance?

The sliding fee scale cannot be used instead of your insurance. NorthPoint’s sliding fee scale should be a last resort. If your employer offers insurance, you will not be eligible for NorthPoint’s sliding fee scale.

Workers Compensation Bills

We will submit bills on verified open workers’ compensation claims. If we cannot verify the claim, your health insurance will be billed.

General Billing

Depending on the services provided, you may receive one or more bill. In addition to your NorthPoint Clinic bill, you may receive a statement from Hennepin County Medical Center (HCMC).

Payment Options

NorthPoint Health & Wellness has several payment options for our patients and families. These options include, cash, personal checks or credit cards. We accept Visa, MasterCard and Discover.

If you cannot pay your patient balance in full, it may be possible to establish a payment plan. The term and payment amount is determined according to the amount owed. A Patient Account Representative can assist you in establishing a payment plan. For more information, please call 612-543-2544.
