Obituary information

If you would like to direct gifts to NorthPoint in an obituary, we can help you with suggested memorial gift language:

  • In lieu of flowers, donations in memory of _____ may be made in support of _____ at NorthPoint Health & Wellness Center. Please make checks payable to NorthPoint and mail to: Development Office, 2220 Plymouth Ave N, Suite 1600, Minneapolis, MN 55411 or donate online.
  • The family of _____ requests that memorial gifts be designated for _____ at NorthPoint Health & Wellness Center. Memorial gifts can be made in our online donation form or mailed to Development Office, 2220 Plymouth Ave N, Suite 1600, Minneapolis, MN 55411.

If you have designated tribute gifts to NorthPoint in memory of your loved one to your friends and family, please contact us to let us know. Be sure to include the name and mailing address of whom you would like us to notify regarding memorial donations.
