Health Care Home Certification
NorthPoint has applied for, and is in the process of preparing the necessary materials to become certified as a Health Care Home (HCH). A Health Care Home is “a community-based primary care setting which provides and coordinates high quality, planned, patient/family centered health care promotion and chronic condition management.” In order to meet the Minnesota Department of Health (MDH) Health Care Home certification requirements/criteria, NorthPoint has established a HCH project team and Advisory Committee. A detailed project plan has been developed with a target date of June 1, 2010 to submit the necessary documentation to MDH. The Minnesota Department of Health will review the materials and schedule an on-site visit to verify that NorthPoint meets the defined requirements, and qualifies for certification.
Diabetes Education Program Certification
NorthPoint provides services to new and existing patients diagnosed with diabetes, including weekly education sessions in three different languages. Our Diabetes Self Management Education Program uses established curriculum created by the International Diabetes Center and is certified by the American Diabetes Association Education Recognition Program (ADA ERP). In 2009, NorthPoint was the only federally qualified health clinic to be granted the ADA’s four-year nationally recognized certification.
